HSA & FSA INFORMATION

Flexible spending accounts (FSA) and health savings accounts (HSA) allow you to put aside pre-tax money to use on certain eligible out-of-pocket health care costs.

If your doctor recommends weight loss for a specific condition including but not limited to diabetes, obesity, or hight blood pressure, the Internal Revenue Service (IRS) may allow you to use funds from your pre-tax accounts such as your FSA or HSA to pay with tax-free dollars. 

If eligible, you may use FSA and HSA dollars for the Zivli program and any additional 1:1 coaching you purchase al-la-carte.

We do not require any documentation for you to use your HSA or FSA funds to join Zivli. You simply use your HSA or FSA like a regular credit card.

However, you may want to obtain a letter of medical necessity from your physician for your own records.

Here are the steps:

1. Download the template letter of medical necessity (below) and request your physician fill it out.

2. Use your HSA or FSA card to purchase Zivli.

3. Email [email protected] to request an itemized receipt to keep for your records.
